NYS Requirements for Athletics
For the protection of your child the following NYS requirements must be met before he/she can practice or participate in interscholastic sports each season:
1. The health office must have a recent physical examination (must be within one year of the start of practice for that season).
2. The completion of the Emergency Information and the Medical History Questionnaire. Also, both forms must be signed. The Health Office must be notified if there are any changes in your child’s health status during the sports season.
3. The Greenburgh North Castle Union Free School District also requires a completed Athletic Contract to be signed by the parent, student, and coach before a student athlete may participate in his/her first contest.
4. Upon completion, the School Physician will review forms and render a final “clearance” for student athletes for practice.
